1)  Fill out the Authorization Form for Release of Information below.

2)  Submit your writing that relates specifically to JSW on a floppy disk directly to instructor.

3)  Art work can be handed to instructor.

 I also acknowledge that any information I initially approve of may be retracted from the JSW web site upon my written request.

I understand that my submission may be edited, MUST BE DEEMED APPROPRIATE FOR THE JSW WEBSITE, and is subject to instructor and staff approval.
